WHO CAN MAKE A REFERRAL?

Any health care professional may make a referral to PCC of La.  A family member, friend, church or community member may also make referrals.  Your PCP will be notified of the evaluation.  Once an evaluation is complete,  Palliative Care Consultants will consult with your PCP to determine if palliative care services is best for you and a plan of care will be established.
